Lost Connections

I found this diagram of Lost connections and some of them puzzle me. I must not be watching as close as I thought I was.
So, if anyone has some info on:
Rose being Walt's grandmother?
The Bernard/Hanso connecton?
Jack and Claire being siblings (although I guess that this was shown when Jack's dad went to that house looking for his daughter?)
